1. Why the UDID Card Matters
The Unique Disability ID (UDID) project aims to create a single national database for Persons with Disabilities (PwDs). Possessing a valid UDID card streamlines access to government schemes, school admissions, scribes for board exams, and travel concessions across India.
Under the Rights of Persons with Disabilities (RPwD) Act 2016, 21 benchmark disabilities are officially recognized.
2. Documents Required for Online Application
Coloured passport-sized photograph of the child or individual.
Address proof (Aadhaar Card, Voter ID, or Ration Card).
Identity proof and existing medical assessment reports from certified government specialists.
3. Step-by-Step Registration & Medical Assessment
Visit the official Swavalamban Card portal (swavalambancard.gov.in) and fill out the online application.
Select the designated district government hospital for the medical board assessment.
Attend the clinical assessment appointment. Upon verification, the medical board issues the digital disability certificate and dispatches the physical UDID card.
